There has been much discussion about what, if any, legal and moral duties professionals have to disclose relevant genetic information to the family members of someone with an identified disease predisposing mutation. Here, we present a case report where dissemination of such a genetic test result did not take place within a family. In contrast to previous literature, there appeared to be no deliberate withholding of information, instead distant relatives were unable to communicate relevant information appropriately. When communication was facilitated (...) through the follow-up of a chance remark, the patient was able to avoid planned major surgery, with its attendant morbidity, and her high anxiety levels were much reduced. We believe this case highlights the need for an ongoing debate on how genetic services can best support patients and their families with disclosing relevant genetic information to other family members. We analyse a recent paper in which an alleged devastating criticism of the so called GRW proposal to account for the objectification of the properties of macroscopic systems has been presented and we show that the author has not taken into account the precise implications of the GRW theory. This fact makes his conclusions basically wrong. We also perform a survey of measurement theory aimed to focus better on the physical and the conceptual aspects of the so-called macro-objectification problem.

A brief review of the conceptual difficulties met by the quantum formalism is presented. The main attempts to overcome these difficulties are considered and their limitations are pointed out. A recent proposal based on the assumption of the occurrence of a specific type of wave function collapse is discussed and its consequences for the above-mentioned problems are analyzed.

Stiff Person Syndrome is a rare autoimmune disorder associated with antibodies against glutamic acid decarboxylase, the key enzyme in γ -aminobutyric acid synthesis. In order to investigate the role of cerebral benzodiazepinereceptor binding in SPS, we performed [ 11 C]flumazenil positron emission tomography in a female patient with SPS compared to nine healthy controls. FMZ is a radioligand to the postsynaptic central benzodiazepine receptor which is co-localized with the GABA-A receptor. In the SPS patient, we found a global reduction of (...) cortical FMZ binding. In addition, distinct local clusters of reduced radiotracer binding were observed. These data provide first in vivo evidence for a reduced postsynaptic GABA-A receptor availability which may reflect the loss of GABAergic neuronal inhibition in SPS. (shrink)

SummaryIntense, sustained nursing lengthens inter-birth intervals and is causally linked with low natural fertility. However, in traditional settings, the effects of such nursing on fertility are difficult to disentangle from those of nutrition. Results from an prospective, direct observational study of reproductive function in well-nourished Amele women who nurse intensively and persistently but who also have high fertility are here presented. Endocrine measures show that ovarian activity resumes by median 11·0 months postpartum. Median duration of postpartum amenorrhoea is 11·3 months, (...) time to next conception is 19·0 months, and the inter-birth interval is 28·0 months. Average life time fertility is 6·8. High fertility in Amele women is due both to refractoriness of reproductive function to suckling stimuli, and to maintenance of equivalent age-specific fertility rates across the reproductive life span. Using payment to recruit research subjects is a common practice, but it raises ethical concerns that coercion or undue inducement could potentially compromise participants’ informed consent. This is the first national study to explore the attitudes of IRB members and other human subjects protection professionals concerning whether payment of research participants constitutes coercion or undue influence, and if so, why. The majority of respondents expressed concern that payment of any amount might influence a participant’s decisions or behaviors regarding research participation. (...) Respondents expressed greater acceptance of payment as reimbursement or compensation than as an incentive to participate in research, and most agreed that subjects are coerced if the offer of payment makes them participate when they otherwise would not or when the offer of payment causes them to feel that they have no reasonable alternative but to participate . Views about undue influence were similar. We conclude that human subjects protection professionals hold expansive and inconsistent views about coercion and undue influence that may interfere with the recruitment of research participants and impede valuable research. The third volume of Ordo V in the Amsterdam edition of the Latin texts of Erasmus presents the second half of Erasmus’ commentaries on eleven Psalms. Two of them not only explain the psalms themselves, but also deal with topical questions, viz. the war against the Turks and the disintegration of Christendom.

Background: Older, frail patients with multimorbidity are at an especially high risk for disease severity and death from COVID-19. The social restrictions proved challenging for the residents, their relatives, and the care staff. While these restrictions clearly impacted daily life in Norwegian nursing homes, knowledge about how the pandemic influenced nursing practice is sparse. Aim: The aim of the study was to illuminate ethical difficult situations experienced by Norwegian nurses working in nursing homes during the COVID-19 pandemic. Research design and (...) participants: The research design involved semistructured individual interviews conducted with 15 nurses working in 8 nursing homes in 3 health regions in Norway, within both urban and rural areas. Ethical considerations: Oral and written information about the study was provided before the participants gave their written consent. The transcribed interviews were de-identified. The study was approved by the Norwegian Centre for Research Data. Findings: Four ethical difficult situations were identified: (a) turning the nursing home into a prison; (b) using medication to maintain peace and order; (c) being left alone with the responsibility; and (d) s. impact on decision-making. Conclusions: The nurses’ ethical challenges were intertwined with external factors, such as national and local guidelines, and the nurses’ own internalized factors, which were connected to their subjective professionality. This duality inflicted emotional distress and gave nurses few opportunities to perform nursing in a professionally sound and safe manner. (shrink)

In his recent paper, “What a Real Argument is”, Ben Hamby attempts to provide an adequate theoretical account of “real” arguments. In this paper I present and evaluate both Hamby’s motivation for distinguishing “real” from non-“real” arguments and his articulation of the distinction. I argue that neither is adequate to ground a theoretically significant class of “real” arguments, for the articulation fails to pick out a stable proper subclass of all arguments that is simultaneously both theoretically relevant and a proper (...) subclass of all arguments. In previous work I argued against (i) the likelihood of finding a theoretically sound foundation for the linked/convergent distinction and (ii) the utility of the distinction even if a sound theoretical basis could be found. Here I subject Douglas Walton’s comprehensive discussion of the linked/convergent distinction found in Argument Structure: A Pragmatic Theory to careful scrutiny and argue that at best Walton’s theory remains incomplete and that attempts to fill out the details will run afoul of at least one of (...) the problems adduced above—i.e., result in either a theoretically unsound distinction or a theoretically sound, but unnecessary distinction. George Bowles and Thomas Gilbert claim that illatives such as so, therefore, and hence convey the meaning that the premise confers upon the conclusion a probability greater than 1/2. This claim is false, for there are straightforward uses of these illatives that do not convey the meaning that the probability is greater than 1/2. In addition, because Bowles' and Gilbert's claim is false, a revision of their definition of argument is required.
